Kaz Manufacturing of New York had a vaporizer and this additive they also manufactured to be used with the vaporizer. The label lists all of the ingredients, like oil of camphor, lavender, yellow dye, menthol, etc. It also shows a drawing of the vaporizer at the bottom of the label. I have never seen anything like it. The bottle is in excellent condition, as well as the label and the metal black screw on cap. I don't know if it was ever opened, but it appears to be full to me.
