Includes "Ownership Statement on inside page CONDITION: Front and back cover show signs of wear, corners show very minimal signs of wear. Cover has very small tear at spine at very bottom. Front cover has a handwritten name across center and front cover page is creased at bottom right corner edge, and minor curling on bottom right corner of pages. Staples and pages appear tight, inside pages are clean, not marked, pages 10, 11, 13, 15, 16 and back cover has small tear about half of the way down, along the right hand side edge.
